Quit Smoking Medication Cards

The Quit Smoking Medication Card can be used to supplement the Tear-Off Resource Guide. This card provides a high-level overview of tobacco cessation prescription medication options, and highlights the Michigan Tobacco Quitline. Providers can use this as a discussion tool with patients interested in quitting smoking.

The card can be used as a flat card, or folded in half to form a booklet.
